You have been referred to me for testing of your peripheral nerves in order to help diagnose the causes of your symptoms and the severity of nerve damage, if present.
The results of this testing will help determine the location and characteristics of your problem, and ultimately help to guide the potential treatment of your problems.
In order for us to spend more time productively together talking and evaluating your condition, I hope that you can take the opportunity to fill out the following paperwork before you come to the office.
